- GP practice name and location
The following metadata is also collected automatically to enable auditing of submissions.

How we get the personal information and why we have it
Expression of Interest (EOI) from primary care professionals are collected via an online form
- Availing of our services (e.g. Research Readiness Program)
- Participating in research in the Primary Care Sector
We use the information that you have given us to:
- provide you with information about other relevant research studies that you may wish to participate in
- notify you about changes to our services
Under the UK General Data Protection Regulation (UK GDPR), the lawful bases we rely on for processing this information are:
(a) Your consent. You are able to remove your consent at any time. You can do this by contacting DPO@easternfsu.co.uk
(f) We have a legitimate interest.
How we store your personal information
All information you provide us is stored digitally via Cloud in line with UK GDPR. Where international data processor are used, all appropriate technical and legal safeguards will be put in place to ensure you are afforded the same level of protection as within the UK.
Unfortunately, any information transmitted via the internet is not completely secure. Although we will do our best to protect your personal data, we cannot guarantee the security of your data transmitted via external sites, however once we have received your information, we will use strict procedures and security features to prevent unauthorised access.

You are not required to pay any charge for exercising your rights. If you make a request, we have one month to respond to you.
